Q: Does PCAA have information on its privacy practices?

A: Yes. Please see PCAA Notice of Health Care Provider's Privacy Practice document on this website for more information..

 

 

 

 

Q: Is PCAA a provider for my insurance carrier?

A: PCAA accepts a number of insurance carriers.  One of which is United Health Care.  PCAA accepts private insurance carriers which allow you to select a primary care provider.   For example, if your current insurance is Blue Cross-Blue Shield of Wisconsin, you can see one of the physicians at PCAA.  Please contact your insurance carrier if you have more questions.

 

 

 

 

Q: How can I get my medical records transferred to PCAA?

A: Please see the Authorization for Disclosure of Health Information form.  Print a copy of this at home and return the form either to PCAA or to your previous physician's office.

 

 

 

 

Q: Which hospital does the physicians at PCAA see patients?

A:  The physicians of PCAA see the majority of patients at AMC.

 

 

 

 

Q: How do I contact PCAA to make an appointment?

A: You can call PCAA during business hours at 920-996-1060 to schedule an appointment or see the our Contact page for additional information..

 

 

 

 

Q: With Dr. Kristine Juntunen joining PCAA, when will she begin seeing patients and when can patients begin to make appointments?

A: Dr. Juntunen will begin seeing patients on November 1, 2007.  Patients can now register with Dr. Juntunen and make appointments during normal business hours by calling 920-996-1060.
