Robert M. Makeever, M.D.

Bob, an Illinois native, graduated from the University of Illinois with a degree in instrumental music education. After teaching for two years, he pursued medical training, receiving his medical degree from Washington University School of Medicine and completing his Family Medicine residency at St. Paul-Ramsey Medical Center in Minnesota.

Bob is Board Certified in Family Practice. His professional interests include obstetrics, health promotion, and disease prevention. He is a member of the American Academy of Family Physicians, Wisconsin Academy of Family Physicians and is a diplomat of the American Board of Family Practice.

Bob and his wife, Susan, have five children. His interests include family and church activities, golf, running, cross country skiing, fishing, and gardening.

 

 

 

 

Robert S. Fox, M.D.

Rob is a native of southeast Michigan, moving to Appleton in 1983 to join Appleton Family Practice Associates. He has a thirteen-year-old daughter Callie, and is newly married to Barb Holly-Fox in 1999. Board Certified in Family Practice, he has truly enjoyed the "family" in Family Medicine, considering it a privilege to care for the many individuals and families he has through the years.

His professional interests include adult medicine, preventive medicine, depression, and pharmaceutical formulary management. He has served on the Boards of the VNA, Agape, and chaired the board of his daughter’s Grace Christian School. He currently co-chairs the Pharmacy and Therapeutics committee of Touchpoint HMO. As a member of Primary Care Associates, he joins in a strong commitment to the advocacy of our patients as they access the health care system.

In their free time, Barb and Rob enjoy church activities, travel, bicycling, sailing and skiing.

 

 

 

 

David M. Ebben, M.D.

Dave is a Wisconsin native and grew up in the Kimberly area. He was in practice in Fond du Lac, Wisconsin until January 1993, when he returned to the Fox Cities. He received his medical degree from the University of Wisconsin School of Medicine in Madison, and completed his residency in Family Medicine through the Cedar Rapids Medical Education Program in Cedar Rapids, Iowa.

Dave is Board Certified in Family Practice and has been in practice since 1982. He is a member of the American Medical Association, American Academy of Family Practice and the Wisconsin State Medical Society.

Dave and his wife Lynn have three Children. In his spare time, he enjoys boating, golf, snowmobiling, and spending time with his family.

 

 

 

 

D. Jon Derksen, M.D.

Jon is a graduate of St. Olaf College, Northfield, Minnesota and the University of Minnesota Medical School. He completed his Family Practice Residency at Hennipin County Medical Center in Minneapolis and from there moved to Appleton where he has been in Family Practice since 1975 He is board certified in Family Practice since 1975.

His medical practice is focused on health maintenance and giving his patients the information with which to make their own health care decisions. In addition, Jon and his wife, Sue, have taken annual trips to Chiapas, Mexico where they provide health care information for the indigenous people of that troubled region.

Jon clearly likes the invigorating Midwestern outdoors with its many opportunities for cross country ski racing and mountain bike racing as well as other outdoor athletic activities. He was honored as the year 2000 Badger State Games Male Athlete of the Year. Jon and Sue have three children all of whom are involved in health care professions.

 

 

 

 

Dan L. Heyerdahl, M.D.

Dan graduated from the University of Wisconsin-Eau Claire and received his medical degree from the University of Wisconsin Medical School in Madison. He served his residency in Family Practice at St. Michael Hospital in Milwaukee. He currently is board certified in Family Practice by the American Board of Family Practice and is a Fellow in the American Academy of Family Practice. His areas of professional interest are preventive health care, pediatrics, gynecology, and sports medicine.

He is active in many professional organizations including the American Academy of Family Practice and the Wisconsin State and Outagamie County Medical Societies. He has served on the Board of Directors of the Appleton Family Practice Residency and as Chairman of the Appleton Board of Health.

Dan and his wife Karen have three children. He enjoys his family, golf, boating, fishing and skiing.

 

 

 

 

Jonathan E. Hagen, M.D

Jon graduated from Augustana College, and received his medical degree from the University of Iowa Medical School. He served his residency at the Appleton Family Practice Residency. His areas of professional interest are pediatrics, sports medicine, and wellness.

He is a member of the American Academy of Family Physicians and the Wisconsin Academy of Family Physicians, and also volunteers at the Fox Cities Community Clinic.

Jon’s community activities include teaching at First English Lutheran Church and coaching various sports. He and his wife Joy have four children. He enjoys hiking, bicycling, skiing, and going "Up North."

 

 

 

 

Amy H. Servais, M.D.

Originally from LaCrosse, Wisconsin, Amy received both her undergraduate degree and medical degree from the University of Wisconsin-Madison. Amy moved to the Appleton area to complete her residency at the Appleton Family Practice Residency. She is currently board certified in Family Practice.

Amy is a member of the American Academy of Family Physicians and the Wisconsin Academy of Family Physicians. Her areas of professional interest are women’s health, pediatrics, and sports medicine.

Amy and her husband, Chris, have three children. In her spare time, Amy enjoys spending time with her family, scrap-booking, golf, and attending Badger games.

 

 

 

 

Charles A. McKee, M.D.

 

Chuck grew up in Appleton where he was Little All-American Quarterback at Lawrence University. Following graduation, he received his medical degree from Case-Western Reserve School of Medicine. He completed residencies at the University of Oregon and the University of Colorado.

Chuck was elected to Phi Beta Kappa at Lawrence and Alpha Omega Alpha, honorary medical society at Case-Western Reserve.

Chuck maintains an active association with Lawrence where he is Medical Director and serves as team physician.

Chuck and his wife Lesley have four daughters and a dog. His interests include skiing, running, hockey, cycling, kayaking, sailing, movies, travel, and reading. He also dabbles in trombone and knitting.

 

 

 

 

Douglas R. Meyer, M.D.

Dr. Meyer is a native of Mequon, Wisconsin and earned his undergraduate degree from the University of Wisconsin-LaCrosse.  After receiving is medical degree from the University of Wisconsin-Madison, he completed a medical internship in Grand Rapids, Michigan.  Dr. Meyer then returned to Wisconsin where he finished his residency training at UW Health Fox Valley Family Practice in Appleton, Wisconsin.

He is a member of the American Academy of Family Physicians.  His professional interests include Preventive Healthcare, Adult Medicine, and Pediatrics.

Dr. Meyer enjoys spending time with his wife Erica and two children.  His hobbies include golfing and long distance running, and he has competed in several marathons.

 

 

 

 

 

 

Stephen C. Fuller, M.D.

Steve grew up in North Central Iowa.  He is one of eight children.  His father was a family doctor.  The satisfaction and admiration that his dad received from serving the small rural community enticed Steve and several of his brothers into careers in medicine.

 

All of his college and post graduate training took place in the Midwest.  He went to undergraduate school in Minnesota, receiving a Bachelor of Arts degree in Biology from St. John’s University.  He received his medical degree from Creighton University School of Medicine in Omaha, NE.  He completed his residency in Family Medicine through the University of Wisconsin in Appleton.

 

Professionally he believes in taking care of the entire family and the advantages his specialty offers in continuity of care.  His special interests are in preventive medicine, urgent care, pediatric medicine and sports medicine.  He belongs to the American Academy of Family Physicians and is board certified by the American Board of Family Medicine. 

 

Steve likes to spend his free time outdoors.  He enjoys hiking, fly fishing and bird watching.  He considers himself an avid sports fan.  He especially likes football and basketball.  Steve also finds reading, gardening and woodworking very pleasurable.

Sonya R. Eiben, M.D.

 

Sonya is a graduate of Augustana College in Illinois and received her Medical Degree from the University of Iowa College of Medicine.  After her residency at Iowa Hospitals and Clinics, she practiced in Minneapolis for a short time before moving to Appleton in 1993. 

Sonya is board certified in Internal Medicine and her professional interests include women’s medicine and preventative medicine.  She is a member of the American College of Physicians.

Sonya and her husband, Jim have two children.  In her spare time Sonya enjoys cooking and spending time with her family. 

 

 

 

 

 

Stephen R. Dernlan, M.D.

Originally from Manitowoc, Wisconsin, Steve received both his undergraduate degree and medical degree from the University of Wisconsin-Madison. He completed his residency at the State University of New York in Syracuse NY, and has been practicing in Appleton since 1989.

Steve is Board Certified in Internal Medicine. His areas of professional interest are all areas of general internal medicine including diabetes, thyroid disorders, hypertension, cholesterol control and routine preventative care of adults.

Steve and his wife Vicki have 3 children. His hobbies include spending time with his family, church activities, camping, hunting and athletics.

 

 

 

 

 

Kristine Juntunen, M.D.

Kristine was born and raised in Sturgeon Bay, Wisconsin and completed her medical school at the Medical College of Wisconsin, in Milwaukee.  It was also at the Medical College that she completed her residency. 

Kristine is Board Certified in Internal Medicine and her professional interests include medical treatment of obesity and medication management of chronic diseases.

Kristine and her husband have two sons and a daughter.  In her free time she enjoys personal fitness including elliptical training and biking.  She also enjoys spending time snowmobiling and boating.

Michael A. Johnson, M.D.

Mike is a native of Michigan City, Indiana  and moved to Appleton in 1993.  He received his medical degree from Indiana University Medical Center, where he also completed his residency. 

Mike is board certified in Internal Medicine and his professional interests include diabetes management.

Steven J. Hagen, M.D.

Steve is a graduate of Northeast Missouri State University and Northwestern University Medical School.  He served as a medical officer in the Navy on active duty from 1992-1996.  During that time he also completed his General Surgery Internship at Oakland Naval Hospital before serving three years with the First Marine Division at Camp Pendleton, CA.  Steve then came to Wisconsin to complete his residency training at UW Health Fox Valley Family Practice in Appleton.

His professional interests include preventative care across the lifespan, pediatric/adolescent medicine, sports medicine and geriatric medicine.  Steve is a member of the American Academy of Family Physicians and the Wisconsin Academy of Family Physicians.  He also volunteers at the Fox Cities Community Clinic and helps with athletic programs at Fox Valley Lutheran High School. 

Steve and his wife, Jill, have two sons.  In his free time he enjoys helping coach his son’s sports teams and fishing every chance he gets.

Travis L. Kroner, M.D.

 

Travis, a native of Kimberly, Wisconsin, received his undergraduate degree in Biomedical Engineering from Marquette University.  After receiving his Medical Degree from the Medical College of Wisconsin-Milwaukee, he returned to the Fox Valley to complete his residency at the UW Health Fox Valley Family Practice in Appleton. 

Travis is Board Certified in Family Practice and a member of the American Academy of Family Physicians. His professional interests include pediatrics, women’s health and preventative medicine.   

Travis and his wife, Katie, have two children.  In their free time, Travis and his family enjoy biking, fishing, reading and music.

Deb Ralston, M.D.

Deb completed her undergraduate and medical school at the University Of Iowa College Of Medicine, the same state in which she grew up.  She performed her residency at Grant Medical Center in Columbus, OH.  Deb has been practicing family medicine in the Fox Valley area since 2000.

Nichol Hess, P.A.-C

Nichol, a native of Green Bay, WI, graduated and received her Physician Assistant certification from Saint Francis University in Pennsylvania.  She did her undergraduate work at St. Norbert College in Green Bay. 

A Wisconsin certified Physician Assistant, Nichol is also a member of the American Academy of Physician Assistants.

Judy Anderson, APNP

PCAA is proud to welcome Judy Anderson to "our family", as a new internal medicine provider.   

Originally from Michigan’s upper peninsula, Judy moved to the Fox Valley 15 years ago with her husband Jon and two children.  She earned her Bachelor of Science degree in Nursing while attending Northern Michigan University.  She received her certification as a family nurse practitioner at the University of Wisconsin-Oshkosh. 

Judy’s professional interests include women’s health care, preventative health maintenance and the management of chronic diseases.

As an "empty nester", Judy is able to spend more time sewing and reading. She enjoys running, as well as camping and canoeing with her husband Jon. They also frequently travel to Minnesota to spend time with their daughter and to watch their son compete in collegiate cross country.

Megan McMillin, P.A.-C

Megan was raised in a small town in Northeastern Iowa.  She attended Des Moines University in Des Moines, Iowa and graduated with a Physician Assistant degree.  After college, she and her husband, Noel, moved to the Fox Cities area.
